A link to a more updated Client Psychotherapy Intake Form, Limits of Confidentiality, Therapy Cancellation Policy, and the Authorization to Disclose Information Form will be emailed to you in a fillable format after the consultation appointment is scheduled.
If you would like me to coordinate care with another provider (for example, your psychiatrist, primary care physician, etc.), a form similar to this one will also be emailed in fillable format after the consultation appointment is scheduled.
